BrooklynBrooklyn care, explained

Notes for your assessment

What happens before a prescription?

Begin with your reason for seeking care. A useful conversation leaves room for your health history, your questions and a plan for what happens next.

Choose a setting for the conversation

An in-person visit puts you in a room with a clinician, the health professional assessing you. Telehealth uses a screen, phone or online exchange.

For either route, ask how the clinician will gather enough information to assess your concern.

  • In a room: ask who will see you and whether there is time to discuss options.
  • Online: ask how to send records and how to ask your own follow-up questions.
  • For either: ask what would lead to a test or an in-person exam.

A form needs a conversation around it

An online form may collect your history. Ask whether a clinician can come back to you for more detail.

Picking a product on an online medicine list does not provide enough basis for a prescription. The national group of state medical boards took that position in guidance issued in May 2026.

That guidance says a patient’s own follow-up questions need a way to reach the clinician.

That advice does not itself set New York law. Ask how the service meets New York’s own rules.

  • Who reads my answers?
  • How can I ask about a different option?
  • What happens if an online visit cannot answer the question?

Keep a gap after each question so you can add the reply.

Bring the details and plan the next step

A short note can keep the visit focused. Write as you normally speak; medical terms are not required.

  • The concern bringing you to care and any recent changes.
  • The medicines and supplements you take, plus past reactions.
  • Your past care, test results and any pregnancy plans.
  • What you can spend and how you prefer to be reached.

Leave with a next step

Have the plan explained simply enough for you to say it back.

  • What happens if no prescription is the right outcome?
  • Which tests are being considered, and what would they change?
  • Whom can I contact with questions once the visit ends?
  • What is included in the visit fee, and what comes separately?
